Diabetes Prevention and Nutrition continues to be a priority for Colfax County. The estimated diabetes prevalence for Colfax County was 951 adults in 2002 representing a rate of 8.8%; just slightly lower then the state rate of 8.9%. Since Colfax County has been addressing diabetes as a priority we have seen the prevalence of patients drop from 1006 in 2000 to 965 in 2001 to 951 in 2002. Obesity is selected due to its known connection to diabetes and the increased concern at the state and national levels. There is no local data available at this time directly related to obesity. Data will be gathered in the near future for the local schools when they complete their “PANT” project.
Assets in the Community: CCCAC Health Council Diabetes Subcommittee, which was first organized in October 2002, Diabetic Education Specialist in the Family Practice Office located in Raton who also works with Miner's Colfax Medical Center, the Colfax County Extension Office, which offers the Kitchen Creations classes and the Joslin classes, Internal Medicine Offices and Public Health Office.
Needs/Gaps: Knowledge of Diabetes is limited in regards to the need for testing, symptom identification, risk factors, and prevention.
Aim: To increase the spread of information to the general public regarding Type II Diabetes including its relationship to obesity and increase the availability of screening for Type II Diabetes.
Outcome: Residents will take proactive steps to decrease their chances of acquiring Type II Diabetes and patients with Diabetes will take proactive measures to control their Diabetes early.
